I’m a CLTS provider, what do I need to do to order Goally?

Here's the info we'll need from you after you've submitted your prior auth into the Gainwell system:


1) Size and type of tablet you want to order (you can see the choices here: Daily Skills System or SGD-3

2) Caregiver/Parent's Name:

3) Shipping Address:

4) Parent's Email Address:

5) Parent's Cell Phone Number:

6) Prior Authorization #

7) Member ID #


Once you send this info and the prior auth #, we can place the order on our side, then submit a claim using that prior authorization.
